Terms & Conditions

All Examination Request form must be submitted at least 1 week prior to the scheduled exam. If you are unable to fill in each required field, you must contact the office at 212.237.8031 or via email as soon as possible.

To ensure testing accommodations all students must submit a student notification contract form to the Office of Accessibility Services. Students who do not submit a student notification contract form for the current academic year will NOT receive testing accommodations.

Students who are scheduled to have The Office of Accessibility Services proctor their exams are responsible for the following:

  • Fill-out the Examination Request Form within a timely manner;
  • Be on time for the scheduled exams; Lateness of the exam will be deducted from the time that is allowed for the exam
  • If a student needs to reschedule an exam due to an extreme circumstances they must provide documentation to The Office of Accessibility Services.
    Please note: It is at the discretion of the instructor to reschedule exams;
  • Only approved equipment/materials by the instructor will be allowed within the Testing Center.
    No coats, food, backpacks, purses, cellular phones, or other electronic devices are allowed in the Testing Center.
  • All students must abide by the John Jay College of Criminal Justice Academic Integrity Statement.
